We push new code into live enviroments all the time. In my case we are up and running 24/7 so we try to do it during slow periods. However we do conduct 3 levels of testing before the push (developer, QC, and UAT). It just sounds like it wasn't tested fully before the push in this case. On one hand people should have enough smarts to quit running turns once they realize it was glitched. Some of the responsibilty here is obviously in the hands of the developers who pushed it, not all but some for sure.
